• Simply print and complete the worksheet, then mail it to the AMAA. 

Tips For Printing the Form
     • Legal size paper is preferred but will print on letter size as well.
     • For legal size paper (8.5 x 14 inches), and set the printer to landscape (sideways) format.
     • For letter size paper (8.5 x 11 inches), and set the printer to landscape (sideways) format.

     • Set all page margins to no more than one half inch.
